WASHINGTON — The U. S. Food and Drug Administration on Friday approved the first prescription drug to treat pain and inflammation after a study found a link between the drug and arthritis.
An FDA review found that Celebrex, the most commonly prescribed drug in the United States, has a weak link to osteoarthritis and a slightly higher risk of heart disease. Celebrex also had a slight increase in the risk of osteoporosis, the highest risk in a large clinical trial, according to the FDA.
The drug, known generically as celecoxib, is used to treat arthritis and other types of arthritis. It is also used to treat pain and swelling, or to treat chronic pain.
But while Celebrex has shown benefits in helping to reduce inflammation in people with osteoarthritis, arthritis, and other forms of arthritis, the drug does not have any effect on osteoarthritis. The FDA has not issued a decision on whether to continue using the drug.
Researchers found that taking Celebrex for longer than four months was not linked to an increased risk of stroke or heart attack.
“The FDA is reviewing the evidence and is working with the FDA to work out the best approach for patients who need to continue taking this medication,” said Dr. Steven Nissen, a doctor and epidemiologist at Harvard Medical School. The FDA said that it had not received additional information about the risks of Celebrex, but that it had “recommended that the agency conduct a review of the available scientific evidence,” the agency said.
“The FDA has issued a clear warning about the increased risk of osteoarthritis,” said Dr. Marcia Angell, director of the National Institute on Aging. “We are continuing to work with the FDA to determine the best approach to prevent or treat these conditions and to continue the use of Celebrex as needed.”
The study was published in the March issue of the Archives of General Internal Medicine. The researchers included 18,000 adults, ages 65 and older, who were taking either an NSAID or an arthritis drug, an anti-inflammatory drug, or both.
The study was funded by the National Institute of Health, the University of North Carolina at Chapel Hill and the National Institute of Aging, the American College of Cardiology and the National Institute of Dental and Osteoporosis Research.
The researchers were unable to conclude that the drug had an increased risk of heart disease, according to a previous study published in the April issue of the Archives of General Internal Medicine. The researchers said the results may not be definitive because of possible confounding by other risk factors, such as smoking, diabetes, high blood pressure, and alcohol use.
The FDA’s analysis was based on a pooled analysis of all available studies that included participants taking Celebrex and other NSAIDs. The FDA has not issued a decision on whether to continue using Celebrex, but has said that it “will continue to monitor the progress and continue to evaluate the risks and benefits of using the drug.”
